OPSEUJob Title: Social WorkerStatus: Regular Full TimeDepartment: LG LTC SOCIAL WORKProgram: Long Term CareDays, Evenings; 8 Hour Shifts (Subject to Change)Location: Lakeridge Health Long Term Care APWage Schedule: $44.670 - $57.900 per hourPosting Reference: 2500000906This position is open to all qualified OPSEU staff throughout Lakeridge Health commencing 25 March 2025 and closing at 11:59 pm on 1 April 2025.POSITION SUMMARY:Reporting to the Manager of Program Supports, the Social Worker will promote and facilitate a resident-oriented environment and act as a resident/family advocate at the Lakeridge Gardens 320 bed long term care home built on the Ajax-Pickering hospital site in the delivery of excellent care for residents.Having opened early 2022, this new home is part of the government's plan to improve long-term care delivery and capacity in Ontario and in the Durham Region. This state-of-the-art long-term care home sets a new bar for seniors' care, with dining and outdoor space on every floor, a large courtyard, onsite diabetes care and other important innovative features.Just as Lakeridge Health is a beacon in the Durham community, the new long-term care home will be integral to managing and delivering on the needs and expectations of seniors in the Durham community and Ontario.The Social Worker is responsible for:*Designing, developing, implementing, and evaluating the social work programs and services that are designed to meet the ongoing psychological needs of the residents and their families/advocates*Introducing the resident/family to the care community and its operations; assists and supports resident/family with adjusting to living in long term care*Conducting comprehensive psychosocial assessments to determine resident needs*Assisting residents in developing or enhancing coping skills related to the social, emotional, and psychological difficulties he/she presents with*Providing therapeutic services through the facilitation of individual, group, and family counselling for residents and families*Act as a support and an advocate for residents' rights in areas such as independence and control over decision-making, managing and/or maintaining social and family connections, and conflict resolution*Supports residents and families during advanced care planning and treatment plan conversations, including funeral arrangement discussions*Follows up on concerns/complaints that arise to ensure that they are addressed in a timely manner and achieve satisfactory outcomes*Liaises and consults with community partners, company resources and professionals in relation to resident care needs and department activities*Documenting and maintaining accurate records, reports and statistics necessary for program functioning and evaluation, to determine the effectiveness of ongoing social work programs*Adheres to the code of ethics of the profession, in maintaining standards of practice for the profession as defined by the OCSWSSWQUALIFICATIONS:*Master of Social Work Degree*Registered and in good standing with OCSWSSW*A minimum of three (3) years of experience as a licensed social worker in a Long-term Care facility*Demonstrated knowledge of the OCSWSSW Standards of Practice and commitment to maintaining reflective practice standards*Experience working with geriatric populations with specialization in Alzheimer's disease and other dementias, depression, responsive behaviors and other psychogeriatric issues*Experience with assessments and care planning, supportive therapy, crisis intervention and grief counseling*Counselling skills and experience in facilitating individual, group, and family therapy*Demonstrated experience in data analysis, program evaluation and research*Demonstrated knowledge of advanced group theories and interpersonal relations*Demonstrated effective leadership and interpersonal skills*Excellent verbal communication skills*Demonstrated commitment to our core values of Inclusion, Compassion, Innovation, Teamwork and Joy*Demonstrated ability to foster good public relations and effective working relationships*Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-service education sessionsCONDITIONS OF EMPLOYMENT:The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and OPSEU Local 348.Successful External applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months.Accommodation will be provided in all parts of the recruitment and assessment process in accordance with the Accessibility for Ontarians with Disabilities Act (AODA) .
