They are actively involved in assisting the patient in planning and implementing realistic goals of care and treatment. The Diabetes Nurse Educator would work in collaboration within an interprofessional team. In addition to outpatient support, the Diabetes Nurse Educator will provide some corporate in-patient support and education as a knowledge expert resource role. The Diabetes Nurse Educator also will provide regular support group and outreach education at various community venues during both day time and evening.QUALIFICATIONS:* Registration with the College of Nurses of Ontario (in good standing);*Minimum of 2 years' experience as a Registered Nurse*Demonstrated commitment to maintaining Reflective Practice Standards as outlined by the CNO;* Current B.C.L.S;* BScN is preferred;*Certified Diabetes Educator or evidence of working towards certification required*Proven professional development and continuing diabetes care education activities*Proven knowledge of disease entities, anatomy, medication and effects on physiologyand laboratory data to assess, develop, implement and evaluate client specific healthcare plans*Knowledge of adult learning principles and self-management theory*Effective interpersonal communication and counselling skills*Ability to plan programs and develop educational materials/packages to support diabetes education for individuals, client groups, clinicians and community service providers* Demonstrated commitment to our core values of Inclusion, Compassion, Innovation, Teamwork and Joy*Understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff safety, patient safety and environmental safety and participation in regular in-service education sessions*Proven good attendance record with capabilities of maintaining this same standard.CONDITIONS OF EMPLOYMENT:The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and the Ontario Nurses Association (RN Bargaining Unit Local 051).Successful External applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months.Accommodation will be provided in all parts of the recruitment and assessment process in accordance with the Accessibility for Ontarians with Disabilities Act (AODA) .
