OPSEUJob Title: Social WorkerStatus: Regular Part TimeDepartment: Ambulatory Rehabilitation Clinic - ARC NEURO Program: Medicine8 Hour Shifts, Days Schedule (Subject to Change)Location: Lakeridge Health Whitby Wage Schedule: $44.670 - $57.900 per hourPosting Reference: 2400001131This position is open to all qualified OPSEU staff throughout Lakeridge Health commencing 18 April 2024 and closing at 11:59 pm on 25 April 2024.DUTES AND RESPONSIBILTIES: Reporting to the Patient Care Manager, and working within the multi-disciplinary team, the Social Worker will be responsible for the provision of social work interventions/services including: psychosocial assessments, discharge planning, navigation, crisis/trauma/conflict, individual/group counselling, education, facilitating case conferences/rounds, referrals to services, data collection, and assisting patients and their families in adjusting to the crisis and disruptions of their particular health care needs. QUALIFICATIONS:*Master of Social Work Degree; registration, in good standing, with the Ontario College of Social Workers and Social Service Workers;*Minimum of 2 years' experience working in an inpatient or outpatient rehabilitation setting with patients with neurological conditions; *Experience in completing comprehensive social work assessments and interventions*Awareness of stroke best practices and community resources is an asset;*Demonstrated experience in working collaboratively with multi-disciplinary teams and community support agencies;*Ability to work with family members providing services and education as appropriate;*Demonstrated excellent organizational, prioritization, problem-solving and case management skills;*Experience with program development and evaluation; *Demonstrated commitment to our core values of Inclusion, Compassion, Innovation, Teamwork and Joy;*Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-services in this area; *Proven good attendance record with capability of maintaining this same standardCONDITIONS OF EMPLOYMENT:The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and OPSEU Local 348.Successful External applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months.Accommodation will be provided in all parts of the recruitment and assessment process in accordance with the Accessibility for Ontarians with Disabilities Act (AODA) .
