OPSEULAKERIDGE HEALTH SOCIAL WORKER HEALTHY AGING PROGRAM PRIMARY: GERIATRIC ASSESSMENT AND REHABILITATION UNIT (GARU)/RESTORATIVE CARE UNIT (RCU)SECONDARY: CONTINUING CARE EAST & WESTTEMPORARY FULL TIME - 12 monthsSCHEDULE; DAYS, 8 HOUR SHIFTS (subject to change)LAKERIDGE HEALTH WHITBYSALARY: $40.40 - $51.47 per hourOPSEU 2200004351DUTES AND RESPONSIBILTIES: The Social Worker will be reporting to the Patient Care Manager, and working collaboratively with inter-professional team members and community partners with the responsibility for the provision of social work interventions/services including: psychosocial assessments, discharge planning, crisis/trauma/conflict, individual/group counselling, education, facilitating case conferences, data collection, and assisting patients and their families in adjusting to the crisis and disruptions of their particular health care needs. Key components of the role would be to support the utilization of a home first philosophy while working with the patient/family and the inter-professional team on complex patient transitions utilizing short term solution focus interventions. The incumbent will be expected to cover multiple areas/programs requiring social work interventions on various patient care units/committees across all Lakeridge Health sites as assigned. QUALIFICATIONS:* Master of Social Work Degree; registration, in good standing, with the Ontario College of Social Workers and Social Service Workers;* Experience with inpatient mental health and substance use populations, and provision of care through a range of modalities;* Demonstrated skill in mental health assessment, crisis management and knowledge of applicable legislation;* Demonstrated experience in working collaboratively with inter-professional teams and community support agencies;* Experience in delivering comprehensive social work interventions within Medicine, Surgery, CCC, Critical Care, Rehabilitation, and Women's & Children's/ER is required;* Experience providing comprehensive transition management/discharge planning while utilizing an excellent knowledge of levels of care, applicable legislation, and community resources;* Demonstrated ability to organize workload and establish priorities effectively within a fast paced environment with a varied caseload; * Excellent communication skills, orally and written;* Demonstrated effective leadership and interpersonal skills;* Experience with program development and evaluation; * Demonstrated commitment to our core values of Inclusion, Compassion, Innovation, Teamwork and Joy;* Must have a satisfactory performance record; * Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-service in this area; and* Proven good attendance record with capability of maintaining this same standard.CONDITIONS OF EMPLOYMENT:The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and OPSEU Local 348.Accommodation will be provided in all parts of the recruitment and assessment process in accordance with the Accessibility for Ontarians with Disabilities Act (AODA).
