ONALAKERIDGE HEALTHNURSE PRACTITIONERGERIATRIC ASSESSMENT AND INTERVENTION NETWORK (GAIN)POST ACUTE SPECIALTY SERVICESTEMPORARY FULL TIME (Until June of 2021)DAYS/EVENINGS/WEEKENDS; 8 hour shifts, subject to changeLakeridge Health OshawaSALARY: $63.20 - $66.36 per hourONA-2000001554DUTIES & RESPONSIBILITIES:Working as a member of the inter-professional team, the Nurse Practitioner (NP) will provide specialized geriatric services in the GAIN (Geriatric Assessment and Intervention Network) Clinic, an exciting model of person-centred Geriatric Outpatient Care that provides geriatric assessment in the management of frail seniors. The NP functions in an expanded role demonstrating a high level of autonomy and is responsible for: conducting client assessments, diagnosing; ordering and interpreting diagnostic tests; prescribing pharmaceuticals and performing specific procedures within the legislated scope of practice. Working in collaboration with the inter-professional team the NP will ensure a timely response to a client's changing health status and needs; and provide direct patient care focusing on health promotion and maximizing client's safety and function to support frail seniors living at home. We are looking for a strong team player who; demonstrates sound communication skills, will be a champion of person and family centred-care, will actively participate in advocating on behalf of an often vulnerable population, and their caregivers. This position is ideal for a NP with a passion for geriatric care.Hours will require flexibility and may include evening and weekend work. QUALIFICATIONS:*Completion of a recognized Primary Care or Adult Nurse Practitioner program with a minimum of 3 years of geriatric experience required;*Able to prescribe controlled substances as authorized by CNO;*Masters of Nursing;*Current Extended Class certification with the College of Nurses of Ontario;*NPAO Membership;*Certification in Geriatric Nursing from Canadian Nursing Association preferred;*Sound working knowledge of hospital nursing functions, current needs in nursing, health care and geriatric care;*Previous experience in clinical work with geriatric clients; diagnosing and staging of dementia;*Demonstrated ability to exercise independent judgment and to assess situations and problems efficiently and effectively;*Effective verbal and written communication skills;*Excellent interpersonal skills, experience with written documentation, careplans and dictation;* Certificate of completion in PIECES, Montessori Based Dementia Care and GPA is a assest;*Strong interpersonal, organizational, collaborative and problem-solving skills;*A desire to be an advocate for change and work to reducing health inequalities faced by vulnerable populations;*Ability to work well within an inter-professional team and to foster effective working relationships;*Ability to exercise initiative and observe strict confidentiality;*emonstrated commitment to our core values of Inclusion, Compassion, Innovation, Teamwork and Joy;*Must have a satisfactory performance record; *Demonstrates an understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices, including staff safety, patient safety, environmental safety and participates in regular in-service education in this area; and*Proven good attendance record with capability of maintaining this same standard.CONDITIONS OF EMPLOYMENT:The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and the Ontario Nurses Association (RN Bargaining Unit Local 051).Accommodation will be provided in all parts of the recruitment and assessment process in accordance with the Accessibility for Ontarians with Disabilities Act (AODA) .
