ONAONALAKERIDGE HEALTHNURSE PRACTITIONERREGIONAL NEPHROLOGY PROGRAMREGULAR FULL TIME8 HOUR DAYS/EVENINGS/ROTATING WEEKEND SHIFTS (subject to change)Lakeridge Health Oshawa/ WhitbyONA-1900000761POSITION SUMMARY:Reporting to the Director, Regional Nephrology Program, the Nurse Practitioner (NP) will work as a Registered Nurse in the Extended Class (RN (EC)), the NP will formulate clinical decisions and develop adult client care plans for patients with chronic kidney disease (CKD) and patients on hemodialysis. The incumbent will be the key liaison with the Nephrologists, multi-disciplinary team members, primary care, referral partners and the community to ensure seamless transitions, in and out of the nephrology ambulatory clinics, facilitating collaboration among all partners involved in the ongoing care of the client. The NP works in partnership with the Regional Director of the Ontario Renal Network to support the achievement of the deliverables of the Ontario Renal Plan, specifically, facilitating, promoting and enabling dialysis at home. The NP will excel in the four domains of their practice: clinical care, leadership, education and research. The areas of work focus will be varied, across sites, LH Oshawa and LH Whitby, including in-centre hemodialysis across both sites, Transitional Care unit, and the Multi-Care Kidney Clinic. KEY DUTIES AND RESPONSIBILITIES:" Acting as a role model and resource for expertise in clinical nursing practice in the area of CKD to members of the interdisciplinary team;" Providing a specialty-based focus, functions in an extended class nursing role demonstrating advanced assessment skills and sound knowledge of clinical therapeutics to formulate clinical decisions and provide comprehensive patient care for clients with CKD; " Providing internal and external nursing consultation to clients/families, health professionals, groups and organizations related to CKD management;" In collaboration with other members of the health care team, identifying and analyzing the learning needs of Nephrology staff; teaching staff, clients, family and other inter-disciplinary members issues related to CKD and associated comorbid care management modalities;" Facilitating interdisciplinary care by participating in regular team meetings, providing a linkage between the client, members of the interdisciplinary team, physicians and other members of the health care team;" Providing the key liaison role with the client's primary care provider, referral partners and the community to ensure seamless transitions in an out of the Nephrology clinics, facilitating collaboration amongst all partners;" Participating in implementation/evaluation of continuous quality initiatives;QUALIFICATIONS:" Current RN (EC) registration with the Ontario College of Nurses, with a Nurse Practitioner certificate in Acute Care or Primary Care;" Masters of nursing; " Member of Registered Nurses Association of Ontario;" Member of the Nurse Practitioner's Association of Ontario;" Certification as Nephrology Nurse (C. Neph.) is required or working towards;" Demonstrated NP experience and a minimum of 2 years in Nephrology nursing is required;" Strong knowledge of, and demonstrated experience in the care of patients with CKD, specifically patients on hemodialysis and peritoneal dialysis is required;" Knowledge of hospital nursing functions, current needs in nursing and the health care system, including service delivery programs for people with CKD is required;" Demonstrated experience in adult education, health coaching, client self-management and case management skills;" Demonstrated exceptional interpersonal, verbal and written communication skills and judgement when relating to clients, families, co-workers and members of the interdisciplinary team; " Demonstrated ability to build and maintain effective relationships with a diverse range of people, demonstrated negotiation and conflict management skills and ability to work effectively within an inter-professional team;" Demonstrated effective organizational, conceptual and leadership skills; and ability to organize workload and establish priorities effectively; " Demonstrated critical thinking and problem-solving skills and ability to exercise independent judgement and to assess situations and address problems efficiently and effectively; and" Motivated self-directed learner, who take initiative and seeks learning opportunities to enhance client care and professional competence;" Embraces a philosophy of patient and family centered care;" Demonstrated ability to be flexible, adapt to changing clinical situations; " Ability to exercise initiative and observe strict confidentiality" Demonstrated understanding of and commitment to LH's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-service in this area; and" Proven good attendance record, and capable to maintain this standard" Computer literacy, including knowledge of Meditech and computer programs such as Microsoft Office and Internet Explorer an asset;SALARY: $61.49 - $64.75 per hourCONDITIONS OF EMPLOYMENT:This position is open to all qualified ONA staff throughout Lakeridge Health.The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and the Ontario Nurses Association (RN Bargaining Unit Local 051).Accommodation will be provided in all parts of the recruitment and assessment process as required under Lakeridge Health's Employment Accommodation policy.
