The Clinical Practice Leader provides leadership in clinical and professional practice, education and research utilization. As a change agent, expert clinician and collaborator, the Clinical Practice Leader supports and advances the goals and objectives of the hospital to foster excellent patient care. CPL responsibilities are divided as 50% corporate and 50% program responsibilities.The RAI Coordinator will develop, in consultation with Mental Health Program leadership, a staff education plan related to the understanding of the RAI-MH and CAT systems and the health care teams' role in ensuring the capture of accurate and timely patient data. This will include collaborating with Decision Support to abstract and present data in a meaningful way to both inform and empower the care team in improving processes related to these data bases and assisting Mental Health leadership to embed this system in our ongoing Quality Improvement programming.JOB DUTIES & RESPONSIBLITIES:Clinical Practice Leader:* Provides expert clinical consultation for delivery of patient care; * Provides clinical leadership for education, clinical documents, projects, committees, etc. that fall within the CPL's area of expertise. * Provides education for skills and certifications beyond the principle expectations of practice and delegated controlled acts (e.g. Non-violent crisis intervention, Restraints)* Leads and/or consults on development of corporate and program-based education initiatives (including orientation and continuing education)* Plans and delivers both intra- and Interprofessional education offerings, with expertise in recovery focused, trauma-informed practices * Develops accessible education opportunities to support staff participation* Uses a variety of adult learning strategies and evaluation techniques that are well aligned to the intended learning outcomes * Utilizes best available evidence as the basis of all practice, education and professional leadership activities (e.g. practice projects, education classes, policy/procedure development)* Participates in recruitment* Supports staff participation in personal and professional career development.* Assists managers with performance appraisals of staff as required. * Performs clinical practice assessments, and identifies and provides educational support (where indicated) for performance issues and learning gaps. * Consults on and/or develops performance improvement plans with staff as appropriate* Works with human resources, occupational health, and manager to ensure a safe working environmentRAI Coordinator: * Acts as the RAI-MH and CAT resource, and coordinates information with unit manager and program director for both LHAP and LHO sites.* Works with unit manager to establish an effective auditing process to ensure RAI-MH and CAT accuracy and compliance with assessments, coding, and care plan updating.* Assumes responsibility for the computerized data submission process to CIHI and maintains records following each submission within the time frames outlined by the Ministry of Health and Long Term Care.* Works closely with decision support, health records and management to monitor and analyze outputs and reports * Completes hands on assessments of patients for initial, quarterly and discharge submissions* Performs other duties associated with role as assigned.QUALIFICATIONS:* Current registration with the College of Nurses of Ontario;* Baccalaureate Degree in Nursing;* Master's degree strongly preferred, or evidence of working towards; * Completion of Basic Cardiac Life Support (B.C.L.S) program;* Canadian Nurses Association Psychiatric and Mental Health Certification preferred;* Knowledge of RAI-MH and CAT assessment tools;* Proven experience in a practical/clinical role;* Theoretical and practical experience in adult education including educational program planning, development and evaluation;* Demonstrated experience with program planning and evaluation, and managing organizational change initiatives;* Proven effective interpersonal and communication skills;* Proven effective leadership and collaboration skills;* Demonstrated effective critical thinking, prioritization, and conflict resolution skills;* Ability to travel between LH sites;* Compliance with confidentiality requirements;* Understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-service education in this area; and,* Proven good attendance with the capability of maintaining the same standard.
