Their focus is on frail seniors who are at risk of suffering adverse events, loss ofindependence and admission to hospital or long-term care. GEM nurses at Lakeridge Health are members of the Transition To Home Team and provide expert and targeted assessment and treatment services in the Emergency Department; they enhance linkage with hospital and community services and are advocates for the patients they serve. As well, in their role as geriatrics educators and capacity builders within the ED, the hospital and the community, GEM nurses help to build the system's capacity to ensure that patients at risk receive the right care in the right place, at the right time.QUALIFICATIONS:* Registered Nurse in good standing with the College of Nurses of Ontario;* Master's degree in progress or completed;* Member of the Registered Nurses Association of Ontario; * Competent to perform CPR at a Basic Cardiac Life Support level;* Demonstrated excellence in oral and written communication;* Excellent interpersonal, problem solving and teaching skills;* Advanced knowledge of the nursing process, the consultation process program planning,research methodology and crisis management;* Ability to function collaboratively within the interdisciplinary team;* Proven analytical thinking and problem solving skills;* Significant experience in Gerontological Nursing experience and education;* Demonstrated commitment to our core values of Compassion, Innovation, Courage and Trust;* Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety andparticipation in regular in-service education in this area; and,* Proven good attendance record with capabilities of maintaining this same standard.
