DUTIES AND RESPONSIBILITIES:
The Patient Care Specialist (PCS), Regional Nephrology Program, In-Centre Hemo Dialysis and Body Access, reports to the Director, Regional Nephrology Program. The successful candidate will hold a management role and be responsible for: creating supportive learning opportunities and an environment that facilitates growth and development of staff within In-Centre Hemodialysis and the program to ensure continual improvement in clinical practice and quality of client care, especially in the focused area of body access. The incumbent, in collaboration with the two other Patient Care Specialists and Nurse Practioner (NP) on Regional Nephrology, will develop/design, plan and deliver/provide educational programs and activities for in-centre hemodialysis and body access. The PCS will also facilitate the development of the interdisciplinary teams and will support the coordination of shared educational opportunities across the entire Program, working also in collaboration with the NP and the Patient Care Managers. The Specialist will work in partnership with members of the health care team, patient, families, to facilitate excellence in the delivery and provision of comprehensive care.
In collaboration with care team members, Managers, and NP, the PCS will also support the primary work of the body access coordinator and vascular access Nurse, in their day to day care, treatment interventions and support for patients. The PCS will also participate in the design, implementation, monitoring, revising and management of client care and system/clinic processes and procedures to ensure smooth operation of service using in depth knowledge of care delivery systems, regional models, standards and practice such as Clinical Pathways, Medical Directives and Hospital Policies.
Working with the Patient Care Manager of the area, the Patient Care Specialist will ensure appropriate systems and resources are in place to assist in the support to staff as the program continues to meet the strategic initiatives of the Ontario Renal Network, through the work outlined in the Ontario Renal Plan. The Patient Care Specialist will assume the management of the area in the absence of the Patient Care Manager.
QUALIFICATIONS:
„ Baccalaureate degree;
„ Masters level preferred;
„ Management experience preferred;
„ Current registration with the College of Nurses;
„ Proven, recent, significant clinical experience in hemodialysis, vascular access and
„ evidence of recent related continuing education;
„ Nephrology certification is required;
„ Knowledge and experience in vascular/body access is preferred;
„ Knowledge of the principles of adult education and teaching/learning principles;
„ Proven planning, organizational, leadership and prioritization skills;
„ Must be an accomplished change agent;
„ Demonstrated effective assessment, implementation and evaluation skills;
„ Knowledge of relevant legislation, standards, related best practice guidelines;
„ Demonstrated effective written & verbal communication skills;
„ Demonstrated critical-thinking skills;
„ Ability to handle multiple tasks simultaneously;
„ Demonstrated project management and problem solving skills;
„ Current B.C.L.S.;
„ Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-service education in this area; and
„ Proven attendance with the capability of maintaining this same standard.
