WAGE SCHEDULE: $36.395 - $45.096
PROGRAM: Aboriginal Cancer Program
DUTIES AND RESPONSIBILITIES:
The Aboriginal Patient Navigator provides support and advocacy for First Nations Inuit Metis (FNIM) patients and families across the Central East LHIN by facilitating and coordinating access to cancer services (for palliative and supportive care), addressing cultural/spiritual needs, and networking with FNIM and non-Aboriginal partners. The Navigator supports the strategic goals and initiatives of the Durham Regional Cancer Program (DRCC) and Cancer Care Ontario's (CCO) Aboriginal Cancer Control Unit (ACCU) including the expansion of the Palliative Care Education and Mentorship Program to the Aboriginal Health Access Centres (AHAC) to build capacity in palliative and supportive care, and increase symptom management outcomes.
QUALIFICATIONS:
„ recent experience working with and extensive knowledge of FNIM peoples language, culture, customs and traditions, required;
„ University degree in Health or Social Sciences field; or college diploma combined with significant years of work experience in the health and/or social work field;
„ recent experience or knowledge of the health care system and community based resources;
„ demonstrated ability to work in a collaborative team approach;
„ demonstrated ability in building and maintaining relationships;
„ effective interpersonal and communication skills
„ proven systems thinking skills; competence in Microsoft Office computer programs, including Excel and Access;
„ previous work experience in cancer care would be preferred;
„ fluency in a First Nation's language, an asset;
„ must be able to travel and provide own transportation.

„ demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-service education in this area; and
„ proven good attendance record with capability of maintaining this same standard.
