Wage Schedule: $32.710 - $35.430 per hour

DUTIES AND RESPONSIBILITES:

The Indigenous Mental Health & Addictions Outreach Worker provides outreach, consultation, and program development activities within an evidence-based and culturally-informed model of care, to First Nation, Inuit and Metis (FNIM) and Non Status youth and adults.

The Outreach Worker will focus on those with mental health and/or addiction concerns.

The Outreach Worker will collaborate with colleagues and community services to build capacity and promote respect, understanding, trauma-informed, compassion and knowledge from a FNIM cultural perspective.

Primary Duties:

" Provide support and advocacy, including short term transitional case management, for FNIM
and Non Status mental health and addictions clients and their families within Durham Region.
" Develop coordinated care plans and provide appropriate referrals for clients
" Provide crisis management and linkage to crisis intervention services as needed.
" Work cooperatively as a member of the team.
" Participate in the development, implementation and evaluation of services provided.
" Represent the agency through community outreach and everyday interactions with other
community resources, i.e., hospitals, hostels.

Minimum Education and Experience:

" Diploma or degree from an accredited College or University in the Human Services field with a focus in mental health and addictions.


Knowledge:

" Three years of related work experience specifically in a case management capacity, working in
the FNIM community.
" Experience working with and/or lived experience
" Extensive knowledge of FNIM peoples, language, culture, customs and traditions, including a sound knowledge and understanding of the issues that impact on FNIM individuals and the unique dynamics that exist within their cultures.
" Knowledge of local Indigenous communities/networks
" Knowledge of major mental illnesses including; signs/symptoms, medication management etc. , addictions and sexual abuse issues; trauma informed.
" Understanding of Mental Health Act and confidentiality, principles of client centered relationships. Thorough knowledge of key functions of case management including, local special resources and access to services.
" Understanding of intake function including assessment, decision making, service plans, crisis intervention and coping/stress management strategies.
" Understanding of methods of evaluating progress.

" Demonstrated ability to work effectively within a team and independently with minimal supervision, strong time management skills.
" Create a safe, comfortable environment for clients to work within.
" Demonstrated reliability, punctuality and attendance.
" Excellent communication skills, verbal and written.
" Professional, positive attitude.

" Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-services in this area; and;
" Proven good attendance record with capability of maintaining this same standard;
" This position requires the successful candidate to have a valid driver's license and use of a vehicle to transport clients. The successful candidate will be required to provide proof a valid driver's license and 1 million dollar liability coverage upon hire
" Must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past 12 months

The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and OPSEU Local 348.

Successful External applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months.
