This is Lakeridge Health!This position is open to all qualified OPSEU staff throughout Lakeridge Health commencing 24 March 2026 and closing at 11:59 pm on 31 March 2026.DUTES AND RESPONSIBILTIES:The Social Worker will be reporting to the Patient Care Manager of the Positive Care Clinic (PCC) and working collaboratively with inter-professional team members and community partners with the responsibility for the provision of social work interventions/services including: psychosocial assessments, discharge planning, crisis/trauma/conflict, individual counselling, education, data collection, and assisting patients and their families in adjusting to the crisis and disruptions of their particular health care needs. The incumbent will be expected to travel and work at multiple sites, including Peterborough PCC, Aid Committee of Durham Region (ACDR), and other outreach settings daily.QUALIFICATIONS:* Master of Social Work Degree; registration, in good standing, with the Ontario College of Social Workers and Social Service Workers;* Counselling Skills and/or certification in CBT, DBT and Trauma;*Experience working with marginalized populations, navigating ODSP, OW, Trillium and other Medication Coverage.* Demonstrated experience in working collaboratively with inter-professional teams and community support agencies;* Experience providing comprehensive transition management/discharge planning while utilizing an excellent knowledge of levels of care, applicable legislation, and community resources;* Demonstrated ability to organize workload and establish priorities effectively within a fast paced environment with a varied caseload;* Excellent communication skills, orally and written;* Demonstrated effective leadership and interpersonal skills;* Experience with program development and evaluation;* Must have satisfactory performance record;* Demonstrated commitment to our core values of Inclusion, Compassion, Innovation, Teamwork and Joy; and* Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-services in this area.At Lakeridge Health, we value inclusion and diversity.
