This is Lakeridge Health!This position is open to all qualified OPSEU staff throughout Lakeridge Health commencing 26 February 2026 and closing at 11:59 pm on 5 March 2026.DUTIES AND RESPONSIBILITIES:The Ontario Renal Network's (ORN) funded Renal Pharmacist role, will play a critical role in ensuring medication safety in the treatment of patients with chronic and acute kidney disease. The Renal Pharmacist is required to support renal patients that are admitted into hospital, support our pre-dialysis patients in clinic, provide expertise for our home hemodialysis and peritoneal dialysis programs and provide on-going care to patients in our in-centre hemodialysis units.In order to support our Regional Nephrology Program, the Renal Pharmacist will:* Work within a multidisciplinary team to meet the medication related needs of patients which will include distributing medications, maintaining records and medication profiles* Help obtain medication coverage when needed;* Conduct clinical medication reviews with patients;* Liaise with patient, caregivers, family members, and other health care professionals as appropriate to provide medication-related information to or for patients;* Provide drug information for immediate patient care that day;* Participate in research activities which may including preparation and dispensing of investigational drugs, detailed record keeping, including oversight of inventory and participate in research activities such as education and site monitoringQUALIFICATIONS:*Current registration in good standing with the Ontario College of Pharmacists in Part A of the register;*3-5 years of renal Pharmacist experience in pre-dialysis, in-centre hemodialysis, home hemodialysis and/or peritoneal dialysis*Practical knowledge of pharmacology, current best practice clinical guidelines, and ability to interpret lab values; *Strong clinical assessment skills in identifying and solving medication related problems;*Excellent medication counseling skills;*Demonstrated ability to research and provide medication information effectively to patients and other members of the multidisciplinary team;*Demonstrated excellent organizational skills;*Demonstrated ability to work effectively within an interprofessional team;*Demonstrated effective interpersonal and verbal and written communication skills;*Proven ability to exercise independent judgement and to assess situations and problems efficiently and effectively;*Previous hospital experience desired;*Must have a satisfactory performance record;*Demonstrated commitment to our core values of Inclusion, Compassion, Innovation, Teamwork and Joy;*Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-service in this areaAt Lakeridge Health, we value inclusion and diversity.
