This position is open to all qualified ONA staff throughout Lakeridge Health commencing 8 January 2026 and closing at 11:59 pm on 15 January 2026.

POSITION SUMMARY:

This position is a shared role supporting the Outpatient Palliative Care Department and the Diagnostic Assessment Program (DAP) in DRCC. Reporting jointly to the Patient Care Managers of the Outpatient Palliative Care Department and the DAP in DRCC, this position requires flexibility to meet the needs of two busy programs. The position requires a commitment to the principles of optimal customer service, a positive attitude, continuous learning, and quality improvement.

As a member of the Outpatient Palliative Care Team the nursing process will be used in assessing the needs of patients/families, in a hospital and community setting, as they deal with pain and other symptoms related to a diagnosis of cancer, or are faced with the challenges associated with a disease that has reached the end-of-life phase. In doing so the nurse will participate as a member of an interdisciplinary team in the planning and implementation of an identified plan of care. The nurse will be involved in the identification and implementation of appropriate nursing interventions within that plan of care. In addition, the nurse will assist with the education of patients, families and nursing staff in the areas of pain and symptom management, psychosocial support and bereavement counselling and act as a liaison with and a resource to community care providers. As an individual practitioner, the nurse is expected to maintain competence and professionalism relevant to current practice.

As a member of the DAP nursing provides care to patients in the Thoracic, Breast, Prostate, and Gynecological disease sites, as well as the Ontario Lung Screening Program. As a Nurse Navigator in the DAP, the nurse will utilize the nursing process to provide safe, competent care to assigned patients. The navigator's primary role will be to function as a consistent coordinator during the diagnostic phase of care. While working collaboratively within the interprofessional team, the Nurse Navigator will apply his/her knowledge of the health care system and the cancer system to successfully advocate for the patient/family. The navigator will be accountable for facilitating the necessary next steps along the care trajectory.

This position may require some travel and flexible working hours to meet patient care needs. The Nurse Navigator will be working in DAPs situated throughout Central East (Peterborough, Oshawa, and Scarborough). The Nurse Navigator will be part of the DAP nursing team and will be expected to cover for other oncology Nurse Navigators as needed.


QUALIFICATIONS:
" Baccalaureate degree in nursing is preferred;
" Current registration with the College of Nurses of Ontario (in good standing);
" Demonstrated commitment to maintain Reflective Practice Standards as outlined by the CNO;
" Recent experience within the last 5 years in Oncology Nursing;
" Current B.C.L.S.;
" Recent experience in oncology nursing (i.e. on an acute oncology floor, or ambulatory oncology clinic within the past 3 years) is an asset;
" Significant recent experience in Palliative Care Nursing (i.e. on an inpatient palliative care unit, Ambulatory Clinic or Community Nursing within the past 3-5 years) is required;
" Post graduate certificate in Oncology or Palliative care is preferred;
" CNA certification in Oncology (CON(C) or Hospice Palliative Care (CHPCN(C) is an asset, or willing to complete same within 1 year of hire;
" Completion/In Progress of de Souza Institute Patient Navigation Course (mandatory);
" Completion of de Souza Institute Fundamentals in Oncology course or equivalent required;
" Evidence of advanced assessment skills and demonstrated clinical knowledge of cancer treatments and the psychosocial impact of disease;
" Ability to work and perform in stressful and emotionally intense situations, caring for vulnerable patients and their families;
" Experience assisting with interventional procedures, preferred;
" Demonstrated ability to work collaboratively within an interprofessional care team;
" Proven ability to effectively and efficiently coordinate, implement and monitor the plan of care for each patient;
" Excellent communication skills with proven ability to collaborate with patients, caregivers, and members of the health care team both institution and community based;
" Demonstrated telephone assessment and triage skills;
" High degree of professionalism and initiative, with the ability to function independently in a position with limited immediate supervision;
" Proven organizational, critical thinking and teaching skills;
" Ability to work effectively in a fast-paced environment;
" Demonstrated ability to assume a leadership role;
" Membership in RNAO and CANO, an asset;
" Excellent communication skills both written and verbal;
" Proven organizational, critical thinking and teaching skills;

" Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient, and environmental safety and participation in regular in-service education in this area;
