Job Title: Social Worker
Status: Temporary Full Time (5 months)
Department: Assmt Comm Treat Team, LHAP
Program: Mental Health & Addictions
Days, Evenings, Weekends, on-call - 8 and 12 Hour Shifts (Subject to Change)
Location: Lakeridge Health Ajax-Pickering/JCHWC
Wage Schedule: $44.670 - $57.900 per hour

DUTIES & RESPONSIBILITIES
Reporting to the Manager, ACTT, the incumbent will work closely with the interdisciplinary team to deliver treatment, rehabilitation and support services to clients living in the community. The incumbent on the ACT team brings both a specialist and generalist role to the team. The scope of the role includes: practicing case manager of a small group of clients, expertise in the area of assessments and interventions; providing individual and group treatment techniques (i.e. motivational interviewing / harm reduction) as well as assistance with activities of daily living, skill development, psychosocial goals, family support and facilitation of linkages with community partners.

QUALIFICATIONS & REQUIREMENTS:
* Masters degree in Social Work
* Registered with OCSWSSW
* Valid driver's license and access to a reliable vehicle
* 3 years community based clinical experience with individuals with severe mental illness (SMI)
* Knowledge and skill in strength based case management functions
* Experience in the treatment and assessment of severe mental illness, interventions and support with families, and knowledge of benefit entitlements
* Experience in crisis and suicide intervention
* Knowledge of community resources and application of the Mental Health Act
* Demonstrated knowledge of the ACT model, psychosocial rehabilitation / recovery principles and consumer / survivor issues
* Excellent clinical skills in coordinating comprehensive care of hard to engage clients with SMI
* Strong advocate for client's right and preferences
* Evidence of decision making, critical thinking and conflict resolution skills
* Excellent interpersonal communication and documentation skills
* Evidence of strong professional boundaries
* Demonstrated ability to work independently and within a team environment
* Demonstrated initiative and ability to prioritize
* Fluency in a second language an asset
*Demonstrates an understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff safety, patient safety, environmental safety and participates in regular in-service education in this area.
* Demonstrates a commitment to customer service when dealing with patients, colleagues, volunteers and other members of the community and public.

CONDITIONS OF EMPLOYMENT:

The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and OPSEU Local 348.

Successful External applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months.
