OPSEUJob Title: Transition FacilitatorStatus: Regular Full TimeDepartment: Transition Management Program: Healthy Aging8 Hour Shifts; Days, Evenings and Weekends Schedule (Subject to Change)Location: Lakeridge Health Whitby and BowmanvilleWage Schedule: $39.160 - $50.770 per hourPosting Reference: 2400004060POSITION DESCRIPTION:Reporting to the Manager, Transition Management, the Transition Facilitator is accountable for coordinating seamless transitions across inpatient areas to the community. With a focus on the provision of excellence throughout the transition process, the facilitator provides guidance, and mentorship to create efficient and effective transitions. As a key member of the inter-professional team, the Transition Facilitator works collaboratively with all healthcare professionals, patients, and families to support transitions that prioritize patient-centered care. The Transition Facilitator provides comprehensive and timely support, removing barriers to support planning for patients from hospital to the next designation of care, working towards a seamless model of transition.The Transition Facilitator will have demonstrated knowledge regarding community resources and partners within Durham Region; including, but not limited to, Ontario Health @Home, Community Care Durham and Alzheimer Society. The Transition Facilitator will further have knowledge of legislation as it related to Alternative Level of Care, Substitute Decisions Makers Act, Public Guardian and Trustee and Bill 7 in order to reduce unnecessary length of stays and readmissions.QUALIFICATIONS:- Bachelors degree in a healthcare field.- Registration in good standing with relevant professional college preferred.- 2-3 years recent experience in hospital based acute care in a professional field required.- 2 years recent experience in transition/discharge planning required.- Working knowledge of community resources, LHIN Home and Community Care, long term care facilities, rehabilitation and palliative care facilities/programs required.- Experience working with the geriatric population and medical/surgical patients required.- Demonstrated knowledge and skills in transition/discharge planning, conflict resolution and creative problem solving required.- Demonstrated working knowledge of legislation including the Substitute Decision Makers Act, Alternative Level of Care, Public Hospitals Act.- Demonstrated ability to prioritize multiple competing workload demands.- Superior communication and negotiation skills.- Demonstrated ability to work collaboratively as a member of an interprofessional team and build relationships with internal and external partners.In addition, provides visible clinical leadership and demonstrates responsibility, accountability and commitment to the development of a work environment that exemplifies inter-professional and ethical practice; professional autonomous decision making; empowerment; integrative thinking; appreciative inquiry; a shared governance philosophy; and positive recognition through the support of professional development and continuing education opportunities.CONDITIONS OF EMPLOYMENT:The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and OPSEU Local 348.Successful External applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months.Accommodation will be provided in all parts of the recruitment and assessment process in accordance with the Accessibility for Ontarians with Disabilities Act (AODA) .
