OPSEU**Re-posted**Job Title: Social WorkerStatus: Temporary Full Time (12 months)Department: Social Work-CYFPProgram: Mental Health & AddictionsDays and Evenings - 8 Hour Shifts (Subject to Change)Location: Lakeridge Health OshawaWage Schedule: $44.67 - $57.90 per hourOPSEU: 2400003923DUTIES AND RESPONSIBILITIES:Working within an interdisciplinary team the Social Worker will support patient navigation for both the internal and external referrals received by the Child Youth and Family Program. This role will include connecting children, youth and their families to the most appropriate service provider by, liaising with other parts of the health care system and children's mental health system, and by coordinating services and removing potential barriers to meet the needs of the patient. The social worker will support the role of intake, through case management, brief and solution focused interventions, along with consultation. The Social Worker will work within an interdisciplinary team to support the patient's journey through all areas of the Child, Youth and Family Program, including crisis, inpatient, intensive ambulatory program and outpatient. The successful candidate will work effectively with the members of the interdisciplinary team in establishing and monitoring processes to improve intervention and continuity of care for patients, and collaborative relationships with other care providers in the Region. The Social Worker participates in program and other committees.QUALIFICATIONS:* Master of Social Work Degree with a focus on counselling and psychotherapy* Membership in the OCSWSSW; registered health professional;* Minimum of two years of psychotherapy experience with proven ability to perform psychotherapy independently. * Documented training and education in specific therapeutic modalities such as CBT and DBT. Other modalities such as narrative, ACT and EFT would be considered as asset. * Previous clinical experience in the assessment and treatment of children and adolescents with psychiatric disorders within a hospital based service setting;* Demonstrated experience in providing individual, group and family therapy to children and youth with psychiatric illnesses;* Previous clinical experience providing brief service including single session model, solution focused interventions;* Demonstrated knowledge of best practice guidelines pertaining to the treatment of child and adolescent psychiatric disorders including substance use;* Demonstrated skill in mental health assessment and crisis management;* Proven ability to work effectively with other health care providers and community service and schoolBoard providers;* Demonstrated knowledge about relevant community providers, systems, and clinical pathways* Previous experience organizing complex care plans* Proven organizational, prioritization and problem-solving abilities;* Proven to have strong interpersonal skills and written and verbal communication skills;* Demonstrated commitment to our core values of Inclusion, Compassion, Innovation, Teamwork and Joy;* Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-service education in this area;* Must be willing to travel from site to site;CONDITIONS OF EMPLOYMENT:The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and OPSEU Local 348.Successful External applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months.Accommodation will be provided in all parts of the recruitment and assessment process in accordance with the Accessibility for Ontarians with Disabilities Act (AODA) .
