OPSEUJob Title: Social WorkerStatus: Regular Part TimeDepartment: DRCC Social WorkProgram: Drcc Patient CareDays, Evenings; 8 Hour Shifts (Subject to Change)Location: Lakeridge Health OshawaWage Schedule: $44.670 - $57.900 per hourPosting Reference: 2400001289This position is open to all qualified OPSEU staff throughout Lakeridge Health commencing 7 May 2024 and closing at 11:59 pm on 14 May 2024.DUTIES AND RESPONSIBILITIES:As a valued member of the health care team, the Social Worker provides comprehensive psychosocial assessment and one to one counselling services and group education to oncology patients and their families.The Social Worker refers patients to a wide range of community services; assists the patient and family in developing or enhancing coping skills related to the social and/or emotional needs and problems that may accompany illness/disability and assists patients and families with identification of and access to available financial supports that may be required as they receive treatment. The Social Worker works with other members of the Health Care Team, within the Cancer Centre and throughout Lakeridge Health, to support development and implementation of an individualized plan of care for our patients.QUALIFICATIONS:* Master of Social Work Degree and OCSWSSW;* Membership in OASW is considered an asset* Recent clinical experience in oncology and palliative care is strongly desired;* Completion of the IPODE course is considered an asset;* Completion of CBT certificate considered an asset;* Significant experience in both a hospital and outpatient/clinic setting dealing with psychosocial health problems, illness and disabilities;* Knowledge of Meditech and Mircosoft Office Suite is considered an asset;* Proven experience with program development and implementation;* Proven experience with development of patient education materials is considered an asset;* Demonstrated knowledge of health care facilities and community support systems, services, social agencies etc;* Knowledge of adult learning principles and proven teaching/group facilitation skills;* Proven ability to organize workload and establish priorities effectively;* Demonstrated skill in communicating effectively with patients, various institutions, agencies and all levels of staff;* Demonstrated effective leadership and interpersonal skills;* Proven ability to work effectively within a multi-disciplinary team;* Proven to have strong initiative and problem solving abilities;* Demonstrated effective written, verbal and listening communication skills;* Must have a satisfactory performance record;* Demonstrated commitment to our core values of Inclusion, Compassion, Innovation, Teamwork and Joy;*D emonstrates a commitment to customer service when dealing with patients, colleagues, volunteers and other members of the community and public;* Demonstrated understanding of and, commitment to Lakeridge Health's comprehensive safety program and practices including staff, patient, and environmental safety and participation in regular in-service education in this area; and,* Demonstrated pattern of good attendance with the ability to maintain this same standard.CONDITIONS OF EMPLOYMENT:The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and OPSEU Local 348.Successful External applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months.Accommodation will be provided in all parts of the recruitment and assessment process in accordance with the Accessibility for Ontarians with Disabilities Act (AODA) .
