They specialize in geriatric care to assess, plan interventions, support transition planning using a Home First Philosophy, implement and evaluate care of the elderly persons with complex geriatric health problems in collaboration with patient, family, interprofessional teams and with community partners. Their focus is to assist a diverse population of elderly persons to achieve optimal independent functioning, reduce Alternate Level of Care days and ED visits. GEM Nurses observe for trends and potential system improvements and initiates quality improvements as needed. The GEM Nurse role includes hospital wide geriatric capacity building and reporting of quarterly regional metrics.
