LAKERIDGE HEALTH Long-Term Care Home

Position: Occupational Therapist
Program: Long-Term Care
Location: Lakeridge Gardens LTC Home
Status: Temporary Full Time - 6 months
Shifts: Days; 8 hour shifts (subject to change)
Salary: $36.85 - $50.55 per hour

POSITION SUMMARY:

Reporting to the Resident Care Manager, the Occupational Therapist will work closely with residents to help improve their activities of daily living at the multi-disciplinary home team at the Lakeridge Gardens 320 bed long term care home built on the Ajax-Pickering hospital site in the delivery of excellent care for residents.

Having opened in early 2022, this new home is part of the government's plan to improve long-term care delivery and capacity in Ontario and in the Durham Region. This state-of-the-art long-term care home sets a new bar for seniors' care, with dining and outdoor space on every floor, a large courtyard, onsite diabetes care and other important innovative features.

Just as Lakeridge Health is a beacon in the Durham community, this new long-term care home is integral to managing and delivering on the needs and expectations of seniors in the Durham community and Ontario.

The Occupational Therapist is responsible for:

* Overseeing and participating in the delivery of quality occupational therapy to residents
* Collaborating with the care team to conduct resident assessments upon admission and developing individualized treatment plans, as well as monitoring resident progress and evaluating and adjusting care plans as needed
* Communicating treatment plans and goals with the care team, including providing direction and guidance to the OT/PT Assistants
* Documenting resident care plans and progress notes, and keeping accurate records
* Providing effective leadership and expertise by researching and keeping up to date on current Occupational Therapy trends, standards, and techniques
* Working in current standards of the Ministry of Health and Long Term Care and all other relevant by-laws, legislation, acts and regulations

QUALIFICATIONS:

* Master's of Science Degree in Occupational Therapy or equivalent
* Registered and in good standing with the College of Occupational Therapists of Ontario
* Certified in the Montreal Cognitive Assessment (MoCA)
* A minimum of three (3) years of experience as a licensed Occupational Therapist in a Long-Term Care facility
* ADP authorization for Mobility/seating/wheelchair and splinting experience, is mandatory
* Must be physically capable of performing the daily activities associated with patient care and occupational therapy treatment
* Effective verbal and written communication skills
* Strong interpersonal, organizational and problem-solving skills
* Must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past 6 months
* Demonstrated commitment to our core values of Inclusion, Compassion, Innovation, Teamwork and Joy
* Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-services in this area
* Proven good attendance record with capability of maintaining this same standard

The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and OPSEU Local 348.
