JOB SUMMARY:
Reporting to the Clinical Director of the Cancer Program, the Patient Care Manager will lead and support the daily operations of the ambulatory clinics at the Durham Regional Cancer Centre (DRCC).

The Patient Care Manager will be required to facilitate the implementation of evidence informed practice to achieve the delivery of excellent, high quality patient care across the departments within this portfolio while collaborating with other Patient Care Managers to enhance the quality of care across the cancer journey. The Patient Care Manager, as an integral member of the cancer centre leadership team, will be responsible for the ongoing monitoring and evaluation of performance metrics and associated performance improvement initiatives.

The Patient Care Manager is responsible for the leadership and direction of clinics for oncology patients being seen in consultation, treatment monitoring, follow up and supportive care while leading an interprofessional team of RNs, RPNs, and clerical staff. While working collaboratively with all members of the health care team, the successful incumbent will use their effective planning, budgeting and problem-solving skills, to ensure: the development of unit and program processes; the management of staff; and to develop/support business goals including operational efficiencies and optimize interdisciplinary team functioning.

Building sustainable relationships with internal and external stakeholders is a key expectation. The Patient Care Manager will work collaboratively with all members of the interdisciplinary and interprofessional health care team among departments and services throughout Lakeridge Health, the Central East region and cancer centres across the province to support and inform a system of care that is inclusive, comprehensive and reflective of current standards of care delivery.


QUALIFICATIONS:

* Current registration, in good standing, with the College of Nurses of Ontario or other regulatory college;
* Baccalaureate degree in Nursing, Health Administration or other applicable program;
* Master's degree or current enrollment in a Master's program (preferred)
* A minimum of 3 to 5 years of leadership and/or management in a clinical operational environment is required;
* A minimum of three years of clinical experience in oncology is preferred;
* Excellent leadership skills in the areas of finance, human resources, quality improvement, change management and evaluation;
* Advanced knowledge in utilizing quality management data for monitoring/identifying trends, impacts and making suggestions for facilitating/implementing action plans;
* Proven knowledge of and ability to work with Collective Agreements and Labour Relations issues and demonstrated knowledge of relevant legislation, workplace safety standards, quality standards and related best practice guidelines;
* Strong understanding of the provincial Cancer System and Quality Based Procedures Funding in Cancer Care, an asset;
* Working knowledge of the Durham Region, and the Central East region and the relevant stakeholders involved in cancer care;
*Excellent leadership skills that incorporate the ability to build and coach a motivated team;
* Excellent organizational planning, assessment and evaluation skills with demonstrated problem-solving and critical thinking skills;
* Excellent communication (written and verbal) and interpersonal skills with the ability to work effectively with internal and external healthcare partners;
* Demonstrated commitment to ongoing professional development;
* Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-service education in this area;
