CUPELAKERIDGE HEALTHRAI COORDINATOR (RPN) MENTAL HEALTH REGULAR PART TIMESHIFTS 4,8 & 12 HOUR DAYS/EVENINGS/WEEKENDS (SUBJECT TO CHANGE)LAKERIDGE HEALTH OSHAWASALARY: $32.26 - $34.06 per hourThe RAI Coordinator will develop, in consultation with Mental Health Program leadership, a staff education plan for improved understanding of the RAI-MH and develop and monitor a strategy for on-time, accurate, patient assessment. Working with mental health teams you will support the integration of RAI-MH Clinical Assessment Protocols (CAPS) into care planning and present outcome data in a meaningful way to both inform and empower the care team in improving processes related to these data bases and assisting Mental Health leadership to embed this system in our ongoing Quality Improvement programming.JOB DUTIES & RESPONSIBLITIES* Acts as the RAI-MH resource, and coordinates information with unit manager and program director for both LHAP and LHO sites.* Devlopes Works with unit manager to establish an effective auditing process to ensure RAI-MH accuracy and compliance with assessments, coding, and care plan updating.* Assumes responsibility for the computerized data submission process to CIHI and maintains records following each submission within the time frames outlined by the Ministry of Health and Long Term Care.* Works closely with decision support, health records and management, to monitor and analyze outputs and reports outcomes and findings.* Monitors and assists with the completion as needed Completes hands on assessments of patients for initial, quarterly and discharge submissions* Performs other duties associated with role as assigned.QUALIFICATIONS* Current registration with the College of Nurses of Ontario;* Canadian Nurses Association Psychiatric and Mental Health Certification preferred;* Minimum of 2 years experience in Inpatient Mental Health * Proven expertise in understanding and completing RAI-MH. * Theoretical and practical experience in adult education, preferred. * Demonstrated experience with program planning and evaluation, and managing organizational change initiatives, preferred.* Proven effective interpersonal and communication skills;* Proven effective leadership and collaboration skills;* Demonstrated effective critical thinking, prioritization, and conflict resolution skills;* Ability to travel between LH sites;* Compliance with confidentiality requirements and * Proven good attendance with the capability of maintaining the same standard.CONDITIONS OF EMPLOYMENT:The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and C.U.P.E Local 6364.Accommodation will be provided in all parts of the recruitment and assessment process in accordance with the Accessibility for Ontarians with Disabilities Act (AODA) .
