Location: LAKERIDGE HEALTH AJAX-PICKERING
Status: Temporary Part time - until March 31, 2023
Shifts: Days/ Weekends - 8 Hour Shifts; subject to change

This position is open to CUPE staff throughout Lakeridge Health commencing October 28, 2022 and closing at 11:59 pm on November 4, 2022.
Reporting to Activation Team Patient Care Manager, the Activation Team PSW's will work with patients on a daily basis to enhance and optimize their functional status. The ultimate goal of the Activation Team is to encourage consistent participation in daily activities for patients, which will in turn promote improved outcomes.
Duties and Responsibilities:
The Activation Team seeks to assist patients at risk of functional decline to improve or maintain their functional status in preparation for discharge from hospital, and to assist with a smooth transition to their discharge location. As such the Activation Team PSW will collaborate with Physiotherapists (PT), Occupational therapists (OT), Occupational Therapist Assistants and Physical Therapist Assistants (OTA/PTA), as well as department based Nursing Staff (RN/RPN) in order to meet the goals of the Activation Team.
Activation Team PSWs will review patients for appropriateness and will assist in activation of patients such as with engagement in activities of daily living and mobilization within the room, in the hallway, and up to chair. The PSW will be required to accurately document all care provided. Weekend scheduling is part of this 7 day a week Team role, and the PSW may need to participate in other duties as required.
The PSW will assist in the maintenance of optimal safety standards to ensure the safety of patients and other staff.
The PSW will follow the occupational health and safety requirements of the position and report any injury or occupational hazards to an immediate supervisor.
Qualifications:
" Successful completion of a 1 year Personal Support Worker program from an accredited Community College or educational institution accredited with the National Association of Career Colleges (NACC);
" Previous experience in acute care or rehabilitation environments is preferred;
" Demonstrated compliance with and knowledge of safe work practices;
" Proven communication and interpersonal skills as well as the ability to follow both oral and written instructions;
" Ability to exercise sound judgment and to function in an independent, self-directed manner
" Demonstrated empathy, concern and interest in providing quality care and services for patients
" Physical ability to perform the essential duties of the job;
" Must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past 6 months;
" Current CPR certification;
" Demonstrated commitment to our core values of Inclusion, Compassion, Innovation, Teamwork and Joy;
" Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-services in this area; and;
" Proven good attendance record with capability of maintaining this same standard;
