This program has been developed to meet the principles of access and consistency while providing quality care. The ultimate goal of the program is to provide care that is coordinated and navigated along an agreed care path with all information available to patients as needed, and thus, minimizing the time intervals along the full continuum of care.The DAP provides care to patients in the Thoracic, Breast, Prostate, Colorectal, and Gynecological disease sites, as well as the High Risk Lung Cancer Screening Pilot.DUTIES AND RESPONSIBILITIES:Reporting to the Program Manager and Program Coordinator, the Nurse Navigator will utilize the nursing process to provide safe, competent care to assigned patients. The navigator's primary role will be to function as a consistent coordinator during the diagnostic phase of care. While working collaboratively within the interprofessional team, the Nurse Navigator will apply his/her knowledge of the health care system and the cancer system to successfully advocate for the patient/family. The navigator will be accountable for facilitating the necessary next steps along the care trajectory. This position requires some travel and flexible working hours to meet patient care needs. The Nurse Navigator will be working in DAPs situated throughout the LHIN (Peterborough, Oshawa, and Scarborough). The Nurse Navigator will be part of the diagnostic assessment program nursing team and will be expected to cover for other oncology Nurse Navigators as needed.QUALIFICATIONS:* Baccalaureate degree in nursing is preferred; * Current registration with the College of Nurses of Ontario (in good standing);* Current B.C.L.S.;* CNA certification in Oncology or eligible to write and with the commitment to do so within one year of hire;* Completion/In Progress of de Souza Institute Patient Navigation Course;* Completion of continuing education in the area of oncology nursing;* Minimum of three (3) years related experience within the last five (5) years caring for patients and families experiencing cancer;* Evidence of advanced assessment skills and demonstrated clinical knowledge of cancer treatments and psychosocial impact of disease;* Experience assisting with interventional procedures, preferred;* Demonstrated ability to work collaboratively within an interprofessional care team;* Experience with the development and implementation of effective education programs targeted at patients and families;* Proven ability to effectively and efficiently coordinate, implement and monitor the plan of care for each patient;* Excellent communication skills with proven ability to collaborate with patients, caregivers, and members of the health care team both institution and community based;* Demonstrated telephone assessment and triage skills;* High degree of professionalism and initiative, with the ability to function independently in a position with limited immediate supervision;* Demonstrated commitment to our core values of Compassion, Innovation, Courage and Trust;* Must have a satisfactory performance record;* Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient, and environmental safety and participation in regular in-service education in this area; and* Proven good attendance record with capability of maintaining this same standard.SALARY: $33.23-$47.57 per hourCONDITIONS OF EMPLOYMENT:This position is open to all qualified ONA staff throughout Lakeridge Health.The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and the Ontario Nurses Association (RN Bargaining Unit Local 051).Accommodation will be provided in all parts of the recruitment and assessment process as required under Lakeridge Health's Employment Accommodation policy.
