Non-UnionLAKERIDGE HEALTHCOMMUNITY TRANSITION LIAISON COORDINATORPOSITIVE CARE CLINICCASUAL8 HOUR DAY SHIFTSLakeridge Health WhitbyUN.EX.-1600000820 DUTIES AND RESPONSIBILITIES:As an integral member of the Positive Care Clinic, the successful candidate will be responsible for providing a wide range of clerical duties including; booking appointments, answering the telephone and accurate message taking and distribution, facilitating the registration process, filing, documenting and maintaining a variety of statisticalinformation, data entry and writing reports, order entering lab requests, preparation and maintenance of records, brochures, pamphlets, ordering office supplies and performing other duties as assigned. The Community Transition Liaison is the first contact for clients and with the support of registered staff coordinates referrals to other services where appropriate, provides support for family members, partners, and significant others, maintains clinical records and works effectively with community partners. QUALIFICATIONS:* Community College diploma in Social Sciences (e.g. Human Services, Social Services Worker) and/or equivalent combination of education and experience within a healthcare setting;* significant experience related to HIV and Immunology clinical work;* certified Ontario Telemedicine Network scheduler;* Plexus certification;* knowledge and understanding of Ontario Trillium Drug Program, Ontario Disability Support Program and Ontario Works;* experience liaising with pharmacy representatives and data collection related to clinical trials;* intermediate level of computer skills, utilizing Microsoft Word, Excel and Outlook(tested) * sound knowledge of ethical and legal issues;* demonstrated knowledge and ability to work effectively with other health care providers, community support system/services, and social agencies;* proven organizational, interpersonal and problem solving skills;* strong knowledge base understanding the social, psychological, cognitive, emotional and spiritual aspects as it relates to HIV/Hepatitis C patients;* demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff safety, patient safety and environmental safety and participation in regular in-service sessions;* proven good attendance with the ability to maintain this same standard.
