Non-Union LAKERIDGE HEALTH Job Title: Senior Project Lead (MDS/NRS) Status: Temporary Part Time (3-4 days per week)Program: Post-Acute Specialty ServicesLocation: Lakeridge Health Oshawa home site (plus LH Whitby)Wage Schedule: $43.803 - $47.723 per hour base (in lieu of benefits and vacation not included in base)Posting Number: UN.EX.-1600000598This position is open to all qualified applicants effective May 27, 2016 and closing at 11:59pm on June 3, 2016.DUTIES & RESPONSIBILITIES:The Senior Project Lead (MDS/NRS) will be based primarily in the Post-Acute Specialty Services (PASS) program, reporting to the PASS Clinical Director with a dotted line reporting structure to the Director, Health Information Program. The Project Lead will work closely with the PASS Leadership, our Quality and MDS Data Clinical Coordinator, PASS staff, Information Technology, Decision Support leadership and staff and other identified stakeholders (CIHI and HINext Analysts) to review and improve processes around the capturing, recording, validating, correcting and submitting of CCC-MDS and Rehab-NRS data in compliance with MOHLTC and CIHI regulations. To complete this work, the Project Lead will develop a project work plan that outlines the activities involved with 1) completing a current state analysis 2) identifying current gaps, barriers, technical, safety and quality issues and 3) designing and implementing a quality improvement plan to address identified issues. The Project Lead will also develop, in consultation with PASS leadership, a staff education plan related to the understanding of the CCC-MDS and Rehab-NRS systems and the health care teams' role in ensuring the capture of accurate and timely patient data. This will include collaborating with Decision Support to abstract and present data in a meaningful way to both inform and empower the care team in improving processes related to these data bases and assisting PASS leadership to embed this system in our ongoing Quality Improvement Programming. QUALIFICATIONS:" Regulated Health Professional (with knowledge of both complex continuing care and rehabilitation inpatient services) is required " Certification, courses or other formal education on MDS/NRS is required" Demonstrated Project Management experience and training " Proven, recent, significant experience and demonstrated knowledge working with the Complex Care Reporting System (CCRS) and the National Rehabilitation Reporting System (NRS)" Understanding of software used in CIHI submission processes i.e. TREAT."
