LAKERIDGE HEALTH Long-Term Care Home Position: Manager, Behavioural Supports, NPProgram: Long Term Care Location: Lakeridge Gardens Long-Term Care HomeStatus: Full TimeSchedule: Days; Evenings as needed; On-Call; 8-hours shifts (subject to change)Salary: $55.245 - $69.057 per hourPosting: UNEX 2200002985POSITION SUMMARY:Reporting to the Senior Director of Healthy Aging and in collaboration with the inter-disciplinary care team, the Nurse Practitioner is responsible or specialized geriatric services in our multi-disciplinary home team at the Lakeridge Gardens 320 bed long term care home built on the Ajax-Pickering hospital site in the delivery of excellent care for residents. Having opened in March 2022, this new home is part of the government's plan to improve long-term care delivery and capacity in Ontario and in the Durham Region. This state-of-the-art long-term care home sets a new bar for seniors' care, with dining and outdoor space on every floor, a large courtyard, onsite diabetes care and other important innovative features.Just as Lakeridge Health is a beacon in the Durham community, the new long-term care home will be integral to managing and delivering on the needs and expectations of seniors in the Durham community and Ontario. Duties and Responsibilities" Provides comprehensive primary care to residents" Conducts person-centered health assessment/physical of residents on admission with specified time frames" Complies with applicable provincial legislation pertaining to long-term care, Long-Term Care Homes Act 2007 & Regulation 79/10" Participates in regular focused resident and family care conferences as required;" Manages the care of residents by providing pharmacological, complementary and/or counseling interventions" Collaborates and consults with physicians, Director of Care and personal care nursing staff, interdisciplinary team and external resources regarding resident plan of care;" Provides leadership and participates as part of the interdisciplinary team in quality improvement initiatives;" Participates as part of the Senior Leadership teams and participates on the Home Medical Advisory committee, Professional Practice Committee and Infection Control along with other home committees;" Provides formal and informal teaching and coaching in the management of clinical care ;QUALIFICATIONS:" Masters of Nursing or equivalent;" Registration with the College of Nurses of Ontario as Registered Nurse (Extended Class);" Completion of a recognized Primary Care or Adult Nurse Practitioner program;" Minimum of 3 years of geriatric nursing experience (preferably in Long-Term Care);" NPAO membership;" Certification in Geriatric Nursing from the Canadian Nursing Association preferred;" Able to prescribe controlled substances as authorized by the CNO;" Advanced studies in areas of palliative care, pain and symptom management, falls prevention, chronic wounds, dementia care, incontinence, and other geriatric-related care is highly preferred" Previous experience in clinical work with geriatric clients; diagnosing and staging of dementia;" Certificate of completion in PIECES, Montessori Based Dementia Care and GPA is a asset;" Innovative critical thinking with excellent communication, public speaking and problem solving skills" Knowledge of theoretical frameworks and research application" Seeks out opportunities to maintain competency and continued learning" Must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past 6 months;" Demonstrated commitment to our core values of Inclusion, Compassion, Innovation, Teamwork and Joy;" Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-services in this area; and;" Proven good attendance record with capability of maintaining this same standard;CONDITIONS OF EMPLOYMENT:Accommodation will be provided in all parts of the recruitment and assessment process in accordance with the Accessibility for Ontarians with Disabilities Act (AODA).
