Status: Regular Part Time
Shifts: Days/Evenings/Nights/ Weekends - 12 hour shifts (subject to change)
Salary: $34.24 - $49.02 per hour

POSITION SUMMARY:
Reporting to Nursing Leadership, the Registered Nurse will provide excellent nursing care to residents at the Lakeridge Gardens 320 bed long term care home built on the Ajax-Pickering hospital site, Lakeridge Gardens.
This new home is part of the government's plan to improve long-term care delivery and capacity in Ontario and in the Durham Region. This state-of-the-art long-term care home sets a new bar for seniors' care, with dining and outdoor space on every floor, a large courtyard, onsite diabetes care and other important innovative features.
Just as Lakeridge Health is a beacon in the Durham community, the new long-term care home will be integral to managing and delivering on the needs and expectations of seniors in the Durham community and Ontario.
DUTIES AND RESPONSIBILITIES:
* Assessing the individual nursing care needs of residents in the Home
* Coordinating the development and ongoing evaluation of resident care plans with the interdisciplinary team
* Planning, supervising and coordinating the work of Registered Practical Nurses and Personal Support Workers
* Completing and ensuring residents' care assessments, service plans and documentation are up to date and changes are communicated to Nursing Leadership
* Assisting with scheduling of residents' care conferences and participating in conferences
* Working closely with Nursing Leadership to facilitate residents' move in and transfers
* Ordering, and safe keeping, of drugs and narcotics and administering in compliance with all regulatory requirements and Lakeridge policies and procedures
* Facilitating and providing resident/family education
* Supervising resident self-medication assessment and audits
* Supervising and delegating care and medication administration responsibilities to Unregulated Care Providers in compliance with applicable regulations and Lakeridge policy and procedures
* Planning, coordinating, managing and evaluating the resident's care plan in collaboration with the resident/substitute decision maker, physicians, nursing and interdisciplinary team and contracted service providers
* Coordinating the nursing team's resident care assignments to ensure efficient and timely delivery of services to residents
* Responding to and managing significant changes in residents' condition and or medical emergencies
* Coordinating emergency response in absence of Nursing Leadership and Executive Directors
* Responding to resident/family concerns and ensuring appropriate action is taken and or concern communicated to Nursing Leadership and other departments
* Assisting Nursing Leadership in the coordination, implementation and monitoring of Infection Prevention and Control, Health and Safety and Clinical Programs and Quality Plans for the Nursing Department
* Coordinating the scheduling and assignment of nursing students from approved education institutions
* Upholding and promoting the organization's values and philosophy relating particularly to ethics, morality, and integrity as set out in Lakeridge's Code of Conduct
* Completing all other tasks as assigned
QUALIFICATIONS:
* Registration with the College of Nurses of Ontario as a Registered Nurse (in good standing);
* Current B.C.L.S;
* Related experience in a geriatric or Long Term Care environment is preferred;
* BScN preferred;
* Demonstrated commitment to maintain Reflective Practice Standards as outlined by the CNO;
* Demonstrated knowledge of and compliance with safe work practices;
* Must possess strong written and oral communication, critical thinking, care coordination, time management and customer focus skills;
* Demonstrated empathy, concern and interest in providing quality care and service for residents;
* Physical ability to perform the essential duties of the job;
* Must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months;
* Demonstrated commitment to our core values of Inclusion, Compassion, Innovation, Teamwork and Joy;
* Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-services in this area; and;
* Proven good attendance record with capability of maintaining this same standard;

The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and the Ontario Nurses Association (RN Bargaining Unit Local 051).
